We are looking to appoint an Elderly Care Physician with an interest in Acute Frailty. The post holder will support 'front door frailty' service and is supported by frailty Advance care practitioners who prepare patients who meet criteria and start a Comprehensive Geriatric Assessment in ED.
There are opportunities to develop sub specialty services such as surgical liaison, dementia care, falls and community / intermediate care services. This post will contribute to the management of the general medicine take through a contribution to the on call rota.
The successful applicant will work with existing Care of the Elderly Consultants to deliver a comprehensive service for frail and complex patients. There is a DME strategy which describes plans for development of a full range of DME services on both acute hospital sites from acute frailty / front door; ward based care for people with frail and complex needs; sub specialty service development and includes development of interface geriatric services. We have a silver phone service hosted by LDH that runs from 0800- 2000 7 days a week held by Frailty ACP's and supported by the Acute frailty consultant to offer advice, guidance and support in senior decision making for ambulance paramedics, GP's and community clinicians eg matrons. This post supports the realisation of some of these ambitions to support Frailty service development at Bedford.
Main duties of the job
* Responsible for the continuing care of acute elderly and medical inpatients based on frailty/ Care of Elderly ward.
* Carry out regular ward rounds, outpatient clinics and hospital consultations.
* Supervise junior medical staff.
* Participate in the Multidisciplinary Team meetings where appropriate.
* To develop an agreed subspecialty interest as agreed with the Trust and complimentary to the Trust's strategic plans and that of local Commissioners.
* Participate in the management of the service.
* The appointee should attend clinical governance meetings and contribute to the Service Lines audit programme.
* The appointee will recognise and support the need for flexibility in duties to ensure reliable provision of a high quality expeditious service for our patients.
* All consultants will be expected to support trust service improvements and engage with an improvement culture.
* An on-call rota system for junior doctors is in place to comply with the EWTD requirements.

Job responsibilities
The successful applicant will work with existing Consultants to provide a comprehensive service for elderly patients. The successful applicant would be encouraged to develop a specialist interest in order to complement the existing care of the elderly service.
It is anticipated that the successful applicant will take responsibility for elderly patients on the short stay ward and participate with on-call commitments for unselected admissions, which is 1 in 18 during the week and 1:9 for the weekend.
The Trust, along with Milton Keynes University Hospital NHS Foundation Trust (as well as other health and social care partners) have embarked on a strategic change programme known as the BLMK Integrated Care System (ICS). As a result of this, the successful candidate may be required to adapt their job plan in the future to work across more than one site within the area; this may also include delivering aspects of care in community settings which supports the Trust's vertical integration ambition.

Person Specification
Qualifications
* Medical Degree
* MRCP or equivalent
* MD or equivalent Higher Degree or evidence of relevant research
GMC Registration
* Full GMC Registration (GMC Licence to Practice is mandatory before employment commences)
* Entry onto the GMC Specialist Register via CCT (or within 6 months of receipt of CCT at time of interview) or CESR (must be on Specialist Register at time of applying)
Clinical Experience
* Evidence of thorough and broad training and experience in Elderly Medicine
Clinical Effectiveness
* Experience of guideline appraisal
* Participation in relevant local audits
* Participation in relevant national audits
Teaching
* Has attended Teaching the Teachers or similar course
Personal Skills
* Ability to co-operate in a democratic team environment
* Flexible attitude, team player with good time management
Personal circumstances
* Residence within 10 miles by road or up to thirty minutes travelling time of the hospital for purposes of on-call emergencies
* Fit to undertake the role with satisfactory Occupational Health clearance
* Satisfactory DBS clearance

* Able to take full and independent responsibility for clinical work and delivering service without direct supervision
