The Stokes Primary Care Network (PCN) is looking for an enthusiastic and highly motivated Pharmacist to join our team. We value the important contribution that clinical pharmacists make to the team. We have been forward-thinking and ambitious in the way that we have developed the Pharmacist role to provide interesting and varied work that fully supports our patients. This has included work to allow pharmacists to develop in areas that are of interest to them and to provide services that would previously have been provided by GPs and long-term condition nurses. We are keen to provide development and stretch in this role and see the pharmacists as incredibly important elements of the wider clinical team.
Main duties of the job
Working with our prescribing team, pharmacy technicians, pharmacists, GPs and nurses, the pharmacist will: conduct medication reviews and reconciliations; respond to safety and dashboard alerts; manage clinic letters and discharge summaries; support patients with long term conditions (e.g. management of HRT, statins, hypertension and diabetes medication); respond to medication queries; lead on prescribing projects; support QOF, and provide specialist information to other clinicians.

From 1 Apr 25, the Stokes PCN will be formed by the Stokes Medical Group following the merger of the 2 existing PCN Practices: Bradley Stoke Surgery and Stoke Gifford Medical Centre. The 2 practices have worked closely over the last 5 years to share experience and ideas and develop new ways of working. We are excited to become a new and larger organisation that will provide care to 40,000 patients with more growth expected.
The Medical Group will have a team of 6 pharmacists, 3 pharmacy technicians and a prescribing team that will largely manage all medication-related work. Scale will bring opportunity to specialise further and develop additional pharmacist-led patient services.
The post will be based at and work from Stoke Gifford Medical Centre although some PCN work will be required.
Job responsibilities
JOB PURPOSE: To provide pharmaceutical support to the Practice to support safe and effective prescribing under the guidance and direction of the Lead GP. Prescribe medication for patients within agreed protocols. Provide pharmaceutical advice to patients to enable them to get the best from their medicines.
KEY RESPONSIBILITIES:
1. To reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.
2. To provide pharmaceutical medication review and health education advice to patients at the designated practices by face to face, telephone or by notes-based review as agreed with the practice.
3. Make clear and comprehensive records of any interventions in patients EMIS records so that they are available to other practice staff.
4. To provide pharmaceutical input to long-term conditions clinics as appropriate.
5. To hold medication review clinics and facilitate implementation of changes with appropriate follow-up monitoring.
6. Print FP10MDA instalment prescriptions to ensure they meet legal requirements following the agreed protocol (scripts to be signed by GPs and not by pharmacist).
7. To develop and run pharmacist-led clinics to monitor the effects of medication by checking clinical signs and symptoms where medicines are the main intervention (e.g. minor ailments, anticoagulation, asthma, diabetes or blood pressure).
8. Support processes for repeat prescriptions for care homes that are looked after by SGMC.
9. To participate in continuing professional development and work towards an Independent Prescribing qualification.
10. To contribute to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes.
11. To act as a source of medicines information for all of the practice team and patients (e.g. around doses, side effects, adverse events, possible alternatives).
12. To improve prescribing practice through educational support for all prescribers within the Practice.
13. Work with prescribers, local pharmacists, patients and their carers to arrange dosette boxes / other compliance aids.
14. To lead on areas where changes in evidence require changes in prescribing across patient populations.
15. To support improvements in clinical care through practice-based audits and implementing change.
16. To liaise with community pharmacy regarding transfer from care settings and the use of monitored dosage systems.
17. To provide a telephone helpline for patients with questions, queries and concerns about their medication.
18. To manage and support the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates where required.
19. To monitor adherence to the repeat prescribing policy ensuring repeat medication reviews are conducted at the appropriate intervals.
20. To maximise the potential of the appropriate use of the repeat dispensing service.
21. To promote electronic prescribing services within the practice.
22. To implement drug withdrawals and alerts aimed at improving medicines safety.
23. Support the development of policies, clinical protocols and guidelines in the practice.
24. Draft reports summarising status on issues, appraising outcomes, and providing progress reports for the Lead Medicines Optimisation Pharmacist and Practice GP Prescribing Lead.
25. Collate qualitative and quantitative information and lead appropriate analysis to develop and contribute to projects.
26. Analyse, interpret and present data to highlight issues, risks and support decision making.
27. Liaise with the CCG Medicines Management Team on prescribing related matters to ensure consistency of patient care and benefit.

* Minimum 2 years experience post registration.
* Experience of working in primary and/or secondary care settings.
* Previous work in a multi-disciplinary team.
* Previous experience of using prescribing data.
Qualifications
* Pharmacy Degree.
* Registered with General Pharmaceutical Council.
* Evidence of commitment to continuing professional development.
* Holds or is working towards an Independent Prescriber qualification.
* Member of the Royal Pharmaceutical Society.
