Job overview
We have an exciting opportunity for an enthusiastic Band 6 physiotherapist to join the multidisciplinary team at the Day Hospital. Applicants must be passionate about rehabilitation and support of independence in older persons.
The Day Hospital is an award-winning service, with Practice Development Unit Accreditation. We use an extended scope of practice framework for holistic healthcare management. The Multidisciplinary Service specializes in management of falls, stroke rehabilitation, and Parkinson’s disease, as well as health promotion strategies and treats a wide range of multi-pathologies affecting older persons.
We run a number of exercise and education groups and provide admission avoidance assessment and intervention in patients' own homes. Our outreach team also visits patients in their own homes following discharge from the acute hospital to ensure a safe discharge by managing any immediate risks such as equipment needs and falls home hazard assessments. The post holder will be expected to work confidently across all these streams of work.
A full driving licence is essential for this post. This is a great opportunity to gain new skills and work in an exciting and supportive team dedicated to providing a high standard of care. * Evidence of CPD
* Postgraduate training in care of the elderly/falls rehabilitation
Experience
* At least some experience must have been undertaken in a physical setting within the NHS. Working with older people and in a rehabilitation setting. Previous experience of the supervision of junior staff and students.
