An exciting opportunity has arisen within All-Age Continuing Care for a Funded Nursing Care Practitioner to join our team. We are looking for highly motivated, compassionate, and skilled RN’s who can work autonomously and collaboratively in a busy role to support the team, individuals, and their representatives in the assessment and review process of NHS Continuing Healthcare (CHC) and NHS-Funded Nursing Care (FNC). You will be responsible for line management and support of the Assistant Practitioners, whilst ensuring that comprehensive, person-centred assessments are undertaken so CHC and FNC eligibility can be determined.

Main duties of the job
1. To work within the NHS Continuing Healthcare team to support the Clinical Lead to establish and maintain robust systems to implement current policy and guidance relating to Funded Nursing Care.
2. To use experiential knowledge and clinical skills to ensure that assessments and reviews for FNC and outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS Continuing Health Care and NHS Funded Nursing Care.
3. Facilitating smooth patient flow across all aspects of the patient’s journey including liaison with primary care, secondary care, community venues and specialist services.
4. Supporting and giving direct supervision to Assistant Practitioners as required.
5. To work with the wider All Age Continuing Care team to develop knowledge and skills of processes, policies, and procedures to aid in professional development towards further progression in CHC.

Detailed job description and main responsibilities
1. Managing competing demands to deliver on outcomes within agreed timeframes.

4. Ensuring compliance with safeguarding as outlined in the organisations safeguarding protection policies and procedures. The post holder will have, or acquire through training provided by the organisation, the appropriate level of safeguarding and knowledge, skills and practice required for the post.
5. Ensuring that any infection prevention and control issues are reported to the line manager/Infection Prevention and Control.
6. As required and with under the direct supervision of the Clinical Lead support with specific projects/reports.
7. Update, maintain, organise, gather and analyse information to predict/meet future organisational and team needs by identifying best professional practice.
8. In relation to the CHC process and applications monitor and track risks and issues establishing a proactive resolution and escalation processes in close liaison with the Senior Nurse.
9. Provide coordination of and participate in relevant meetings, reporting attendance and providing information, advice and support where requested.
10. Ensure that data collected in relation to CHC reviews is analysed, reported by the team as appropriate and monitor the processing of data and information.
11. Provide relevant and timely specialist advice and guidance on functional and information matters relating to the Continuing Care Review process.
12. Under direct supervision with the Clinical Lead and as required support the development of internal and external communications where required by regular contact with the teams, stakeholders and communications team.
13. Be able to work within a multidisciplinary team and with family members to ensure the CHC/FNC review is undertaken inclusively.
14. Interpreting national and regional guidance relating to the implementation of the National Framework and subsequent developments within all of the CHC/FNC service.
Person specification
Knowledge, Training and Experience
* Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area.
* Registered with their professional body and hold a clinical qualification eg Registered General Nurse, Registered Learning Disability Nurse or Registered Mental Health Nurse.
* Minimum of 2 years post nurse qualification experience.
* Experience and understanding of evaluating and measuring the performance of health services.
* Comprehensive knowledge of project management and/or health information systems development.
* Working knowledge of Microsoft.
* Experience/ knowledge of CHC/FNC.
* Able to demonstrate understanding of MCA, Capacity, and Consent.
* Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences.
* Evidence of success in efficient and effective project and programme management.
* Skills for communication on complex matters and difficult situations, requiring persuasion and influence.
* Skills for nurturing key relationships and maintaining networks.
Analytical
* Ability to analyse and interpret information, pre-empt and evaluate issues, and recommend an appropriate course of action to address the issue.
* Attention to detail combined with the ability to extract key messages from complex analysis.
