We are looking for an experienced and highly motivated nurse who is passionate about working in the Children’s Rapid Response Team (RR) to ensure high quality, specialist nursing care to children and families in their own home. This role aims to prevent inappropriate hospital admissions and shorten the length of hospital stays while providing greater choice to families regarding their chosen place of care.
The successful applicant will work as part of a well-established dynamic team that provides specialist advice and support to children, young people, their families, and all members of the healthcare teams involved in the child/young person’s care. You will work alongside the Children’s Community Nursing (CCN) and Palliative Care Teams (PC).
Full time, part time, and flexible hours of working will be considered at the interview.
We encourage anyone considering applying for this post to visit the team prior to application and interview and to discuss the role with Team Leader – Rebecca Carless on 07784 007 462 or the Clinical Service Manager - Rachael Williams on 07974 785 098.
You will work very closely with the Co-ordinator for Children’s Community Nursing and Rapid Response to ensure a high standard of care is offered to children/young people. You will support the Clinical Co-ordinator with the clinical and professional management of the Children’s Rapid Response Service (CRRS) to ensure safe and effective care delivery, overseeing the effective and efficient use of skills and resources to ensure the team and the service is organized to meet the needs of the patients and their families.
You will be proactive in promoting early discharge of children and young people already known to the CCN and PC Teams, reducing the length of hospital stay. You will work with the Children’s Rapid Response team and acute teams to identify children and young people that could be cared for at home where it would be clinically safe to do so.
During times of extreme pressure within the acute setting, you will work as directed to facilitate a reduced length of stay for children and young people that are potentially not already known to the CCN and PC Team.
