Job summary This is a very exciting opportunity for an experienced Physiotherapist to support the development, implementation, continuous monitoring and improvement of the Active Together Programme. The overall purpose of this role is to assist in the delivery and sustained success of the exercise prescription, physiotherapy, and physical activity elements of the Active Together prehabilitation and rehabilitation Programme at Airedale. This role includes working under the guidance of the Clinical Specialist Physiotherapist, to assess the physical, dietetic and psychological needs of these patients. To be able to use these findings to develop a personalised, safe and effective physiotherapy/ physical activity programme. This will include supporting behaviour change and promoting healthy lifestyle changes. Other responsibilities will include deputising for the Clinical Specialist Physiotherapist when necessary, and the supervision and training of assistants, students and other less experienced team members. Main duties of the job Support the implementation of the Active Together prehabilitation and rehabilitation services at Airedale NHS foundation trust.To frequently undertake the comprehensive assessment of patients, with a complex presentation and multiple comorbidities. To analyse the assessment to formulate individualised, evidence-based treatment plan, using own clinical reasoning and a range of treatment skills and options to formulate a specialised programme of care. Prioritising this efficiently considering clinical need, priorities and time management.Be an integral part of the Cancer Prehabilitation team working within the MDT to deliver a high-quality effective service, including identification of dietetic and psychological needs of these patients, liaising with the appropriate teams when required.
