Coordinate the care of patients in the Urgent Treatment centre throughout the care pathway with the multi-disciplinary team and clinical services to ensure effective management of patients. Have highly developed communication and examination skills to enable the maintenance of high quality service provision within at times a complex and challenging environment with conflicting priorities. Proactively identify, diagnose and develop treatment plans for patients requiring urgent intervention and treatment and at risk of developing long term conditions. Assist the clinical and nursing teams at all levels in the assessment and prioritisation of patients in the urgent care pathway, and support a team of trainee Advanced Practitioners through leadership and clinical supervision to deliver high quality and effective care. Using advanced clinical assessment skills, autonomously manage patients with complex needs with undifferentiated diagnosis to ensure that optimum patient care delivery needs are met.
