This role will require you to work as part of the new, innovative and forward-thinking Contact Hub, which is situated in South Petherton Medical Centre. The Contact Hub is a central point of contact for patients and facilitates a comprehensive triaging process, to ensure people are seen as soon as possible, by the best person. The centralisation of these functions also allows for a more efficient, responsive, and effective way of managing patients within the local primary care network (PCN), which comprises of the following Somerset based GP practices: Crewkerne Health Centre, Martock Surgery, Buttercross Health Centre in Somerton, Ilchester Surgery in Ilchester, Hamdon Medical Centre in Stoke Sub Hamdon. If you are successfully appointed as a GP at one of the above practiceswithin anyof the above PCN GP practices, you will also be required to work on a on rotational basis at the Contact Hub. You will therefore need access to a car / public transport to travel to the Contact Hub to fulfil the full duties and expectations of this role.Please consider this before applying.
