* Maldon ICT (this team covers 4 areas- Maldon, Witham, South Woodham Ferrers and Burnham on Crouch. You will be expected to cover all areas if necessary; your preference of base will be noted, however not guaranteed).

Detailed Job Description and Main Responsibilities
Within the planned pathway, the post holder will support the effective management of a defined caseload of housebound patients in the provision of quality nursing care, applying evidence-based practice.
Within the unplanned pathway, the post holder will respond according to clinical priority for patients triaged as requiring care outside of the planned pathway.
This includes attending the patient, where possible, within a four-hour period. It is expected that staff working on the unplanned pathway will liaise closely with the planned pathway to hand-over care provided. Working on the unplanned pathway requires the post holder to be proactive in problem solving, considering the patient holistically, including ordering of necessary equipment and liaising with the multidisciplinary team.
