Children's Health Care Support Worker Band 3 Community Children's Nursing Team
Opportunities have arisen for enthusiastic Health Care Support Worker (Band 3) to work in our community children's nursing team in Dudley. You will be part of the service working in children's homes and education settings supporting packages of care for children with complex needs.
Main duties of the job
Following a supportive and in-depth training program, you will care for children with a wide variety of complex health care needs and life-limiting conditions, such as children who have an unstable airway/unstable condition; have complex seizures; are oxygen dependent; fed by enteral feeding methods such as a gastrostomy tube; tracheostomy, catheterisations, and offering support to families.
You will work without direct supervision and be responsible for implementing and maintaining high standards of health care, as planned and directed by our team of registered nursing staff.
It is expected that the post holder will support the cover of the packages of care to enable children with complex needs to attend education. The post holder will also support nurses and families to ensure that practical and emotional support is offered to families and children.
This will involve clinical and non-clinical care and ensure that the post holder works with a wide range of other agencies and professionals including housing, dieticians, clinical engineers, physiotherapy, occupational therapists, and social services.
The post holder will have prior experience working with children with complex clinical health needs and delivering clinical care.
The hours will be term time only, Monday to Friday, 35 hours per week.
Job responsibilities
1. The post-holder will be responsible for the holistic care of children with complex health care needs in the child's own home and special school.
2. Develop and maintain clinical competence and skills by undertaking a competency-based training programme, with assessment and re-assessment at regular intervals.
3. Carry out all relevant forms of care without direct supervision following care plans and the trust policies and procedures.
4. Observe patients and respond to changes in the child's condition, including emergency interventions. Verbally report changes in their condition to appropriate members of staff and document in nursing notes.
5. Provide child-specific care to support the clinical needs of the child in the child's home or community as required. This may include tracheostomy care, enteral feeding support, maintaining airways by using suction techniques, and catheterisations. This list is not exhaustive; the clinical care will be child-specific following competency-based assessments.
6. Recognise the cultural needs of patients and staff and enable them to be fulfilled.
7. Accompany child/young person to designated community places, i.e., school or hospice if BCPHT are supporting a package of care.
8. Following training and clinical competencies, recognise when the child's condition deteriorates and act appropriately.
9. Under direction, support with continence product reassessments.
10. Support children and families with seizure management, including administration of emergency rescue medication, following an agreed care plan and maintaining competences.
11. Following direction from the nurses, administer minor medication as per care plan/policies and as agreed by the RCN.
12. Ensure equipment is maintained and faults are reported following trust policies, and competencies are up-to-date in the use of the equipment.
13. Deliver equipment and consumables as required.
14. Participate in administration of file notes, filing letters, typing, and general administrative duties as requested by the team leader.
15. Participate in the ordering/stocking and stock control of equipment.
16. Promote health, comfort, dignity, privacy, and quality of life whilst supporting and promoting the child's and families' rights.
17. Work within the complex/continuing care framework and protocols, providing and maintaining a high standard of care to patients as delegated.
18. Participate in the planning, implementation, and evaluation of care under the direct/indirect supervision of a qualified or registered Nurse.
19. Work in partnership with the multidisciplinary team according to the Care Plan under the guidance and delegation of appropriate professionals.
20. Communicate effectively with the child and their family, acting as an advocate when needed.
21. Maintain clinical records using Trust documentation policy.
22. Communicate effectively with all professionals to provide a seamless service.
23. Respect the child and young person and families' individual needs.
24. Understand age-related developmental needs of the child, young adult.
25. Help promote and maintain a suitable environment in which the child, young person feels safe and comfortable whilst promoting usual daily activities.
26. Recognise the parents' knowledge and expertise of their child's, young adult's care needs.
27. Respect the child and families' right to privacy whilst a guest in their home.
28. Participate in an initial introductory and ongoing competence-based training program according to the child's changing needs. Acquire an understanding of a range of work procedures and practices, which require expertise, underpinned by theoretical knowledge.
