Job summary The Health Visiting Service in Stockport is seeking to employ a Health Visitor who is enthusiastic and committed to improving the outcomes for children and families in Stockport. The Health Visitors are a key part of our integrated Start Well Team, holding named health visitor 0-5 geographical caseloads to support children to get the best start in life. They work closely with early years providers, GPs, Midwives and Stockport Family colleagues. The service has well established skill mix teams consisting of Health Visitors, Staff Nurses, Early Years Workers and Health Visitor Assistants who together deliver the Healthy Child Programme and early years pathways. They are part of Stockport Family which is an innovative, integrated children's service, designed to ensure strong working relationships between health, Local authority and Education providers, and working alongside colleagues in the Local Authority, Primary Care, voluntary and statutory agencies within newly established Family Hubs. Main duties of the job As a health visitor in a Start Well team you will lead the healthy child programme 0-5 and manage a named health visitor mixed caseload of universal, enhanced and specialised pathway families. You will provide holistic assessment using the my world triangle, and utilise planning skills to identify and address health needs in a defined local population. You will provide early help support to families when required utilising the Early Help Assessment and within a Team Around the Early Years model. You will be accountable for your work and delegate activities as required to other members of the integrated Start Well Team. You will maintain a focus on Public Health priorities, using a community strengths based approach to reduce inequalities utilising family hub networks across the neighbourhood you are working within.
