The Child & Family Wellbeing Practitioner role is to support Health Visitors and nurses to deliver the Healthy Child Programme (0-5), achieving positive outcomes for children and families. This includes delivery of specific elements of the programme and a focus on high impact areas, such as infant feeding and baby massage to support maternal mental health.

Main Responsibilities:
* Carry out work with families as delegated by the accountable Health Visitor and provide feedback on outcomes.
* Support the facilitation of local healthy family drop-ins.
* Plan, deliver, and evaluate virtual health promotion sessions.
* Undertake Ages and Stages 27-month reviews.
* Deliver evidence-based specific "Support bundles" to families under the direction of the Health Visitor.
* Ensure effective communication and appropriate information sharing with partner agencies.
* Work with the Health Visitor in identifying and assessing development and growth.
* Support and contribute to the identification of vulnerable children and children in need of safeguarding.
The Ideal Candidate:
You will have a level 3 diploma in childcare or a NNEB qualification. A caring and approachable demeanor is essential to gain the trust of local families and children. You should be confident using IT systems and have a working knowledge of Microsoft Office packages (Outlook, Word, Excel).
This is a community position, requiring a car driver to travel from home to home or base to home.
