The Senior Clinical Pharmacist will be an experienced clinical pharmacist who acts within their clinical competencies, to develop, manage and mentor a team of clinical pharmacists within Synergy Health PCN. This will include clinical supervision sessions on a regular basis. The postholder will be responsible for providing clinical leadership to the pharmacy team, GP practices and the PCN. The postholder will provide expertise in clinical medicine management, patient medication reviews, manage long term conditions, management of systems for safer prescribing, and manage the health and social care needs of patients within the GP practices included in Synergy Health PCN.
Primary Care Networks (PCN) are local neighbourhoods where all health and social care services work together to deliver a better experience for their citizens.

About us
Synergy Health PCN covers three practices in Gedling, Jubilee Park Medical Partnership in Carlton with a branch site in Lowdham, West Oak on Westdale Lane, Mapperley and Trentside Medical Group in Netherfield and Carlton.
We are a standalone PCN with a lead practice. We have a population of 36,000 patients.
The PCN has care coordinators, social prescribing link workers and are recruiting for a health and wellbeing coach. These roles will work closely together as the PCN holistic care team.
Our vision is to build and develop collaborative working across member practices and stakeholders, to support and strengthen the delivery of Primary Care services and improve health outcomes for the local population we serve.
Job responsibilities
Core Duties
1. Managing caseload of patients with common/minor/self-limiting ailments while working within scope of practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.
2. Undertake clinical medication reviews with patients and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests.
3. Chronic Disease Management (for example, managing hypertension/Type 2 Diabetes/Asthma/COPD patients, polypharmacy review).
4. Act as a source of medicines information for all of the practice team and patients, suggesting and recommending solutions.
5. Focusing on QOF Domains, improving disease registers.
6. Lead on prescription management (systems and processes, ensuring monitoring is being done, support clerical staff).
7. Act as Line Manager for the PCN Clinical Pharmacists to include managing day to day operations and supervision of the Clinical Pharmacists.
8. To develop and facilitate a good working relationship with community pharmacists and other local providers of healthcare.
9. To plan and organise own workload, including audit and project work, and training sessions for members of the practice team, patients, and carers.
10. To maintain registration as a pharmacist and comply with appropriate professional codes.
