Band 6 (Crisis Resolution and Home Treatment Team - Early Discharge Team).
Are you a dynamic, confident and motivated Mental Health Nurse/Occupational Therapist or Social Worker with excellent communication skills with the ability to assess and lead on clinical decision making?

This is an exciting role, working in a fast-paced working environment, within a well-established, multi-disciplinary team. The crisis and home treatment teams offer a high-quality assessment and home treatment service as an alternative to hospital admission, to individuals experiencing acute mental health crisis.
As a service we are looking for individuals who are genuinely warm and engaging and focused on client/customer satisfaction.
Secondment opportunities/fixed team contracts for up to months or permanent contracts.
We also offer full-time and part-time working.
If you are applying for a secondment you must obtain prior approval from your current line manager before submitting your application.
Main duties of the job
1. To work in collaboration with the acute inpatient wards to facilitate safe and positive early discharges.
2. To provide first line assessments of people experiencing acute mental health difficulties and act as gatekeepers to acute admissions.
3. To be able to rapidly assess people with mental health problems who are vulnerable to hospital admission.
4. To provide specialist crisis assessments focusing on “hospital at home.
