Practice Nurse - GP Practice Primary Care
We are seeking to increase our Primary Care Practice Nurse provision at our Practice. Candidates will ideally have an interest in diabetes, but this is not essential. We are looking for part time but would consider more hours. We are a friendly, supportive, innovative, and progressive team and ask for a can-do approach towards delivering high-quality care to our patients. We recognize continuous professional development is important and offer support with this and revalidation. You will be working alongside our Health Care Practitioner with a competent and experienced clinical team to support you. Home visits will be required. We have a fantastic clinical and administrative team for our patient population and our list size is currently expanding.
Main Duties of the Job
To provide nursing services to patients of the Practice including treatment, screening, and advice within the professional competence of the post holder, and to help set up screening clinics. All elements of Chronic Disease Management, Immunisation and vaccination plus cervical screening. Candidates will be expected to help lead and manage local schemes such as Primary Care standards as well as data entry for more complex vaccinations and histories from outside the UK. Extra training will be provided where required and as available. The post holder will work in accordance with the NMC UKCC Code of Conduct.

Job Responsibilities
KEY TASKS OR FUNCTIONS:
1. Provide nursing treatments to patients in participation with the general practitioners or independently to agreed protocols.
2. Provide general and specific health screening to the practice population (within agreed protocols) with referral to general practitioners as necessary.
3. Organise and undertake child health/travel immunisation clinics.
4. Order and maintain vaccine stock and stock rotation.
5. Lead for fridge maintenance and cold chain. Lead for infection control.
6. Responsibility for call and recall and routine/proactive monitoring of patients on Chronic Disease Registers (QOF).
7. Advise patients requesting travel vaccinations and organise and undertake vaccination programme where applicable.
8. Lead on diabetes management if/when qualified to do so.
9. Advise on and administer contraception and depo injections.
10. Coordinate Practice audit as required and lead on infection control management and admin.
11. Advise patients on general health care and minor ailments with referral to general practitioner as necessary.
12. Participate in ongoing practice projects such as secondary prevention of heart disease, PCQRRS, GM Standards, and PCN Incentives such as IIF.
13. Maintain supplies and equipment used, ensure stock control and room audits.
14. Maintain accurate patient records and statistics.
15. Chaperone when required by GP.
16. Take part in Protected Learning Time sessions with other practice team members and attend monthly training sessions.
17. Attendance at Fire, Infection Control, and Resuscitation sessions annually plus all other required training and development.
18. Refer to other members of the multi-professional team as required.
Education and Training
* Nursing Degree or Registered Nurse degree apprenticeship.
* Asthma training.
* COPD Training as required, including Spirometry.
* Health Promotion training.
* Annual Cytology Update.
* Prescribing Course (Optional).
* All routine training such as Information Governance/Safeguarding/Infection Control as required by the Practice.
Communication
A regular daily and/or weekly meeting with GPs on clinical issues.
Regular communication with Practice Manager and Practice team.
Daily review of NHS Email and internal email system.
Daily review of internal and external tasks such as EMIS and Docman.
Regular review of GP Contract, revalidation, top-up indemnity, and NMC membership - requirement on the candidate to ensure this is all up to date.
Key Responsibilities
1. To provide assessment, screening, and treatment services which are relevant to patients and their families, at the Practice, or in the patients' home. Manage and Lead on QOF/IIF/PCQRRS & other projects as required by the Practice.
2. To participate in the administrative and professional responsibilities of the practice team.
3. Participate in data entry as relevant to the role and requirements.
4. To assist in the formulation of practice philosophy, strategy, and policy, and regular update of relevant practice protocols.
5. To assess and monitor patients with chronic disease and recommend referral to GP for change in treatment.
6. To participate in research within the Practice.
7. To liaise with staff in hospitals and community, social services staff, and voluntary organisations, as required to provide an adequate service to the patients.
8. To maintain professional development by attending courses and study days as arranged with the Practice Manager/GP.
9. To co-operate with the University and School of nursing if requested in allocation of student nurses to the practice.
10. To take full responsibility for all areas of any prescribing (if within remit of role and capabilities) and act on any requests or audits from the Medicines Management Team.
11. Ensure Safeguarding duties and responsibilities are undertaken to the fullest extent.
12. Ensure Infection Control measures are adequate and followed to the utmost extent - Lead for this area within the Practice.
13. Ensure continuous professional development and training.
Person Specification
Knowledge and Skills
* Must be RGN.
* Nursing degree or Registered Nurse degree apprenticeship.
* Chronic disease management (ideally with an interest in diabetes).
* Phlebotomy and administering injections are essential.
* Flexible approach towards patient care and recall.
* A positive, proactive attitude and excellent communication skills are essential.
* Team working and good interpersonal skills are essential.
* Experience of QOF and Standards/IIF/PCQRRS is required for this role.
* Experience in a Primary care/community setting.
* IT skills must be of a competent level.
* EMIS Software Experience.
* Diabetes interest.
