Job summary An exciting opportunity has arisen for a full-time permanent band 6 Dietitian to join our forward thinking, supportive and motivated community team of dietitians. The post is based at Ysbyty Cwm Cynon and Dewi Sant Hospital providing dietetic care across Cwm Taf Morgannwg Community sites. The successful candidate will provide dietetic intervention to patients across community settings including Pulmonary Rehabilitation Groups, wards, care homes and patients' own homes as well as regular specialist outpatient clinics and education sessions. The candidate will have a key role in student training and will also work closely with more junior Dietetic staff and Dietetic Support Workers. The successful candidate will share our values and vision of an integrated community dietetic team. You will be joining a forward-thinking group of dietitians who are part of a dynamic Dietetic Department of over 90 dietitians and support staff. The department enjoys a high profile both within the wider Allied Health Professions team and across the Health Board. The department fully supports continuing professional development and regular supervision. With protected sessions in job plans we actively support staff to participate and share learning opportunities and experiences. Main duties of the job To support the provision of health care, to the people of the Cwm Taf Morgannwg University Health Board by providing an efficient and effective dietetic service to a defined specialist caseload of patients. To have clinical responsibility for a mixed caseload made up of home visits, care homes, specialist outpatient clinics, structured group education sessions for adults referred for Dietetic intervention. To support and supervise junior or less experienced dietetic staff across the health board, to develop their skills and experience with complex medical and patients requiring dietetic intervention. To work as part of a multidisciplinary, multiagency team which will include community medical, specialist nurses, therapy staff and other colleagues and agencies both within the hospital and the community.
