Assess, plan, implement and evaluate all aspects of patient care, and develop care plans, using clinical reasoning skills, which may need to be delivered from a range of options. Assess patients holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan. Liaise with GPs, Social services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately. Have the ability to use the remote monitoring facility for Virtual Ward patients, understanding escalation points and when further clinical support is required. Be accountable for work delegated to others within the team. Ensure that Cleo, SystmOne (or other patient records used by the Service) inputting is completed on a daily basis. Will be exposed to bodily fluids, infected material, blood products, therefore must utilise universal precautions and adhere to infection control policies. Process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary. To support clinical activity in Virtual Ward Hub and be competent in all aspects of that hub, interchanging role with the VW Triage Nurse when/if the activity increased in that service. To be responsible for clinical activity in UCCH and be competent in all aspects of that hub, within scope of practice. To be responsible for clinical activity passed by the NCHC SPoC as 2h-UCR and be competence in all aspects of UCR Triaging and allocation, within scope of practice.
