Job overview
An exciting development opportunity has arisen for an experienced nurse or allied health professional to join The Richmond Response and Rehabilitation team as Service Manager. The Richmond Response and Rehabilitation service, established in 2014, is an integrated health and social care service focused on supporting vulnerable adults over 18 years of age in the London Borough of Richmond.
Our pathways consist of:
* Discharge to assess: minimise hospital admission, support hospital discharge and help patients regain their functional independence in the community.
* Community Reablement: supporting vulnerable clients in a domiciliary setting using rehabilitation and reablement goal setting.
* Urgent Community Response (UCR): responding to clients at acute risk of hospital admission, preventing hospital admission and helping to support individuals to remain within the community.
Main duties of the job
If successful, you will be taking the lead in the development of an integrated team culture, bringing together the different skills, experience and knowledge of team members. Our service is composed of a skill mix of nurses, allied health professionals, dieticians and social workers working in the community. You will be central to evaluation, reporting and development of the service and a key contributor to the SWL working groups. You will have the skills required to manage a high functioning, fast-paced team implementing and embedding change.
Person specification
Qualification
* Degree qualification or equivalent level of study/work experience
* Current registration as registered nurse or AHP
Experience
* Leading, managing and motivating staff including performance management and staff development to deliver high quality results
* Managing successful change programmes, working across clinical service areas to improve and develop service delivery
* Coaching, mentoring, conflict management and/or mediation skills
Skills
* Ability to resolve complex clinical issues
* Ability to communicate and negotiate across a range of audiences and organisations
* Ability to build the team to a high-performance level, conducive to professional development and team cohesiveness
Knowledge
* Understanding of current legislation and professional body standards (NMC/HCPC) and their implementation in practice
* Understanding of current and emerging health and social care policy and strategies
* Managing and reporting on performance data and targets.
