This is an exciting opportunity to make a real difference in the lives of individuals recovering from stroke, providing expert assessment and specialist rehabilitation services. You will work as part of a supportive and multidisciplinary team, delivering high-quality care tailored to each individual's needs. The role involves working across the Bristol, North Somerset, and South Gloucestershire regions, offering a varied and engaging caseload in community settings. Key Responsibilities Conduct comprehensive assessments of patients following stroke, identifying their rehabilitation needs. Develop and implement individualized rehabilitation plans to promote recovery and independence. Collaborate with other healthcare professionals to deliver coordinated and holistic care. Provide advice, education, and support to patients, families, and carers. Maintain accurate and timely patient records. Travel between community locations as needed to meet service requirements. Requirements HCPC registration as an Occupational Therapist. Previous experience in stroke or neuro rehabilitation (essential). Experience in community-based roles (essential).
