Overview:
The role of the Care-Coordinator provides a vital service and is a crucial member of the team. The role is fast paced as well as incredibly rewarding. Listed below are some of the key responsibilities – although not exhaustive it was created to give a good snapshot of the day to day actions that a Care coordinator undertakes.
Responsibilities:
* Adhering to Core Values, policies and procedures.
* Applying diversity and equal opportunities policies in the workplace.
* Interviewing prospective candidates for the role of carer
* To ensure all client details, care plans and risk assessments are in place
* To plan to cover holidays, training, and planned absence.
* Take part in the On Call out of hours’ service on a rotational basis.
* Ensure that the care worker rotas are organised and sufficiently well planned to consider travel time and staff skill mix.
* To answer the telephone promptly and professionally, dealing with any enquiries or queries.
* Ensure the provision of high-quality care services to vulnerable people living in their own home.
* To undertake staff supervisions and appraisals
* To liaise with other professionals I.e., GP, Commissioners, district nurses etc
* To work alongside manager ensuring regular reviews of service users are completed in a timely manner
* To ensure all electronic rostering systems are up-to date
* To ensure all travel expense form submitted within the set time frame
Requirements:
* Strong skills in planning, scheduling, prioritising, and time management.
* Ability to establish and maintain effective professional working relationships.
* Has the desire to make a real difference and greatly improve an already well-established business.
* Minimum of 1 years’ experience in a similar role
* Relevant qualification, such as Business Studies or Social Care – Desirable.
* Driving licence and access to own vehicle
* Extremely well organized, excellent planning and prioritising ability with high attention to detail.
* Use of a scheduling / HR / recording system – Desirable.
* Ability to work unsupervised.
* Strong administration skills and ability to manage multiple workloads.
* Self-motivated and flexible, with a willingness to participate in an on-call system for out of office hours.
* Previous Domiciliary Care experience – Desirable.
* Strong Problem-solving skills.
* Knowledge of office – word, excel
