In your day to day you will be expected to do the following:
1. Conduct assessments to identify the needs of older adults.
2. Coordinate care plans and support packages to promote independence.
3. Facilitate hospital discharges and ensure effective transitions to community care.
4. Collaborate with healthcare professionals, families, and community services.
5. Advocate for service users to ensure their voices are heard and needs are met.
6. Maintain accurate records and adhere to statutory guidelines.
7. Support individuals in both community and hospital settings.
Requirements:
1. Fluency in Welsh (essential).
2. Experience in community care and hospital discharge processes (desirable).
3. Strong communication, assessment, and organizational skills.
4. A commitment to enhancing the well-being of older adults.
