Ward: Intermediate Care Therapy
Shift Patterns: 37.5 hours per week core hours
Main duties of the job
1. Conduct specialist occupational therapy assessments, establishing diagnoses and developing tailored treatment plans for patients.
2. Independently manage a caseload of patients, including those with complex needs, with access to senior-level advice and support as needed.
3. Actively contribute to multi-disciplinary team discussions and collaborative care planning.
4. Maintain high-quality documentation in line with Trust and departmental policies.
What you'll need:
* Diploma / Degree in Physiotherapy / Occupational therapy.
* HCPC Registration
* UK NHS Experience
* Post registration experience in rehabilitation and condition management.
