GPConnect PCN is looking to employ an Advanced Nurse Practitioners to support the ourPrimary Care Network team in the delivery of nursing services, working as partof the PCN multidisciplinary team, delivering care within their scope ofpractice to the entitled patient population. Youshould be an enthusiastic and highly motivated ANP with GP Practice backgroundexperience to work at the PCN level. The role will involve working variousshifts within our PCN. The successful candidate will be using advancedautonomous clinical skills and a broad and in-depth theoretical knowledge basein line with the latest RCN guidance on ANP competencies
Main duties of the job
TheAdvanced Nurse Practitioner will work within their professional boundaries;
Providing carefor the presenting patient from initial history taking, clinical assessment,diagnosis, treatment and evaluation of their care.
To undertakesurgeries working in an autonomous role including the management of minorailments, chronic disease, family planning, home visits to include both acutevisits and the management of chronic medical problems in the house-bound
To providetelephone based triage, assessments and consultations.
Demonstratesafe, clinical decision-making and expert care for patients within the generalpractice maintaining clinical excellence.
Work collaborativelywith the multi-disciplinary general practice team to meet the needs ofpatients, supporting the delivery of policy and procedures. In order to work atthis level, NMC requirements for advanced practice must be met.
Develop new andinnovative concepts, models, methods and practices to deliver new and improvedprimary care services to meet the needs of PCN Patient population
Promoteeducation and training to other staff and students, acting as a trainingresource
The post holderwill work strictly in accordance with PCN protocols, policies and guidelinesafter appropriate training and assessment of competences have been completedand satisfied. Flexible working will be required to meet the needs of thepatients group and the service as it continues to develop

GPConnect PCN consists of 5 GP Practices covering approx. 32000 patients. Ournetwork operates with in the city of Coventry.
Thesuccessful candidate will join a GP Connect PCN Team and will be supported by aSenior Team and Regular development sessions, teaching, training andopportunities to upskill will be provided.

Clinical Care Delivery
1. Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making, to establish a diagnosis and management plan
2. Make professionally autonomous decisions for which he/she is accountable
3. Instigate necessary invasive and non-invasive diagnostic tests or investigation and interpret findings/reports
4. To prescribe safe, effective and appropriate medication as defined by current legislative framework
5. Provide safe, evidence-based, cost effective, individualised patient care
6. Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care
7. Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills
8. Refer patients directly to other services/agencies as appropriate
9. Participate in developing and delivering specialist primary care services such as long term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease and hypertension).
10. Work with nursing, medical and health care assistant colleagues to ensure that National Service Frameworks ( Coronary Heart Disease/Older People/Diabetes/Mental Health) are being delivered
11. Work with multi-disciplinary teams within the practice and across the wider CIC, to promote integrated and seamless pathways of care
12. Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery
13. Participate in identification of community health needs and develop patient/family-centred strategies to address them in liaison with appropriate community health professionals and agencies

24. Participate in multi-disciplinary protocol and patient group directions development
25. Identify appropriate opportunities to delegate both clinical and administrative tasks to more junior staff
26. Help the practice operate in a cost effective manner
27. Identify and manage nursing care risks on a continuing basis

30. Work closely with the doctors and administrative managers in the setting up and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets
