Job Summary
To lead on the clinical effectiveness of Pharmacy services, including governance, medication safety and electronic prescribing.
To act as the Trust's Medication Safety Officer. To support local medication error reporting and learning and act as the main contact for NHS England and MHRA.
Main Duties of the Job
1. To be the designated Medication Safety Officer (MSO) for the Trust.
2. To improve the reporting and learning from medication incidents and errors, both within the organisation and outside.
3. To oversee the investigation of all medication related incidents.
4. To provide support for investigators of medication related incidents and, where appropriate, request the provision of a root cause analysis (RCA) report.
5. To work closely with the pharmacy governance lead on all aspects of patient safety relating to medication usage.
6. To drive improvements in data quality from Trust Safeguard reporting system for local and national learning.
7. To report on these to the appropriate national/local body as required.
8. To liaise with, and produce information and reports for, appropriate internal and external bodies such as MHRA and NHSE.
9. To develop, monitor and implement the Trust strategy around medication safety through clinical leadership, taking the lead role in setting the agenda and workplan of the Safe Medication Practice group and acting as vice-chair.
10. To drive change and improvement within the Trust regarding the safe usage of medicines, based on intelligence and investigation from medication incidents, and national benchmarking and learning.

3. To lead on the development and delivery of effective clinical governance relating to medication usage and medication safety initiatives across the Trust.
4. To be responsible for leading the clinical pharmacy service to a division, ensuring that the service is provided at the highest possible standard within the available resources.
5. To develop the service to meet the needs of patients and clinical specialties within the Division.
6. To advise on the management of drug expenditure within the Division to ensure optimisation of medicines usage.
7. To lead on and co-ordinate clinical or practice research and audit within Pharmacy. To lead on teaching across the Trust relating to medication usage and clinical pharmacy.
8. Support the education and training of pharmacists.
Person Specification
Qualifications Essential
* Masters Degree in Pharmacy
* Registered with the GPhC
* Evidence of commitment to CPD
* Registered independent prescriber
* Postgraduate qualifications in Clinical Pharmacy or equivalent
Experience Essential
* Extensive experience as a qualified hospital Pharmacist Management experience at band 8a level or above
Skills and Knowledge Essential
* Able to work as part of a team.
* Evidence of leadership skills/abilities.
* Ability to manage equipment resources within the Pharmacy Department.
* To be able to manage own service within the given financial/staffing constraints.
* Excellent written and oral communication skills.
* Ability to confer complex information to clinical staff and patients.
