Short term services are recruiting for a Support Worker to join our team on a part-time (36 hrs) permanent contract where you will provide care and support to people in the community with a focus on maximising independence, and delivering a high standard of care to a variety of people, including end of life care.
Following a period of acute ill health or hospitalisation, you will undertake short term care as defined in care & short term service plans or delegated tasks from Therapist/Nurses, and contribute to the assessment process and ongoing monitoring and review of people's goals, reporting any changes to a person's medical condition.
This is an exciting development of services to help prevent avoidable hospital admissions working alongside intermediate care teams and other community teams.
You will be working as part of a team, which will include lone working within the community across Torbay and South Devon to provide integrated health and social care, working closely with other organisations.
Key Responsibilities:
* Provide support to service users with personal care, domestic care, social needs, and clinical needs in accordance with the care plan, and within the limits of own competencies and responsibilities.
* Observe and evaluate the service users' response to the care plan and communicate changes in need and/or circumstances to the appropriate member of the service in a timely manner.
* Provide a flexible service which will include shift patterns between 0800 hrs and 2200 hrs, 7 days a week.
* Provide reports on service users using the service IT system, while complying with the Trust Policies and Procedures.
* Contribute to a safe working environment for self and others and report situations accordingly in line with Trust Policies and Procedures and other partner organisations.
* Undertake training relevant to the post and in line with personal development and review.
* During times of reduced work, provide support to other teams linked with the service.

Full training in social care reablement is provided with a uniform and smartphone. You will be paid for each shift regardless of contact time, and travel time is paid at the full hourly rate with a generous mileage allowance. Shift enhancements are paid for evenings, weekends, and bank holidays.
There is the opportunity to rotate through various different settings and career progression opportunities, such as the assistant practitioner role.
