The Intensive Support Service has been developed to provide bespoke supported living services in the Bury area of Greater Manchester to adults over the age of 18 with learning disabilities, autism, mental health needs, and complex needs.
The service will operate 24/7 using a trauma-informed and case management model of care to provide support and opportunities for increased independence in the community for three adults. We have developed the new and exciting role of Support Co-ordinator to provide operational leadership of the service in a dynamic and sensitive way.
The adults accepted into the service have been identified through the Transforming Care Agenda.
Staff members recruited will initially be required to work dynamically between the Registered Office, service, and current care provision to ensure service users experience a well-planned transition into their new home.

You will support the Service Manager to lead the staff team to deliver the highest standards of support, enabling service users to develop and enjoy well-being, quality of life, and community connections. You will promote and reinforce a culture of responsive, person-centred practice and active support across the service. Strong working relationships will be built based on respect and unconditional positive regard. You will hold a case load of at least one of the three individuals living in the service with a clear focus on coordinating all aspects of person-centred support and record-keeping.
