What the Job Involves Your responsibilities include planning and co-designing individual support plans, ensuring regular monitoring and reviews. You will manage social care provision, including your own caseload, while adhering to national and service procedures. Key tasks include undertaking risk assessments, communicating effectively in diverse situations, and representing the department in multi-disciplinary forums. You will work collaboratively with other professionals to meet legal frameworks, and participate in assessing mental capacity and best interests when necessary. Additionally, you will be responsible for maintaining high standards of documentation and ensuring compliance with data protection and confidentiality regulations.
