Job summary We are looking for a motivated, dynamic, and flexible band 7 Occupational Therapist for a fixed term position within East Cheshire Trust Transfer of Care Hub (TOCH). The Home First Occupational Therapist role is funded as part of the discharge investment scheme supporting the implementation of the Home First model across Cheshire East, due to its proven success supporting people to return home and reducing care hours the scheme has been extended until March 26. Hours per week: 37.5 (part time hours will be considered) Main duties of the job The main objective of the Occupational Therapist is to work in collaboration with inpatient and community therapies, Care4CE Reablement service and Right at Home Care provider to support discharge from hospital or a community bed. Completing timely home assessment and put in place adaptations and equipment to reduce risks and support people to return home. The Home First Occupational Therapist completes follow up reviews post discharge and links closely with the Community Connectors and Social Workers to ensure any identified needs are met and supporting people to be as independent as possible. The Home First Occupational therapist will also provide education, training, and development of care to support goal setting, overcoming barriers such as carer anxiety, engagement and training to support independent daily living, though techniques, equipment and small aids. To act as an Advanced Clinical Specialist responsible for the triage, assessment and treatment of patients admitted to the medical and surgical services providing advanced level assessment and intervention for patients with complex needs. To provide advanced level clinical advice and work as part of the wider multi-disciplinary team to support integrated working for this patient group, including support for patients in D2A beds.
