Job summary A post has become available within our dedicated Urgent and Intermediate Care team and will be an amazing opportunity for anyone with a passion and interest in Hospital Avoidance/Home First Health Care. The successful post holder will provide personal care for a range of vulnerable and frail adults within their own homes and in designated bed based areas. The bed base is within our diverse Community Division and will involve residential and respite facilities with all care taking place under the supervision of our highly skilled Urgent and Intermediate Care Nursing & Therapy staff. The successful candidate will deliver and assist with low level therapy programmes that have been set by senior therapy staff, with the opportunity to provide constructive feed back to the senior staff on any issues and progress concerning the individual client/patient. Main duties of the job The post holder is responsible for the delivery of clinical care and support under the supervision of a registered nurse/therapist to a defined population. Supported by a Nurse or therapy practitioner, they will deliver care within the boundaries of their role. They will work effectively and collaboratively within the team to meet the needs of patients, support the delivery of policy and procedures and provide clinical leadership. Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.
