An exciting opportunity has arisen for a Community Matron to join our dynamic Hexham Community Nursing team in Northumberland. The Community Matron will be responsible for the identification of vulnerable patients who are at risk and those frequently admitted to hospital through proactive case management. This role involves collaboration with Specialist Nurses, medical colleagues, Palliative Care, Intermediate Care, Social Care, and Allied Health Professionals to reduce admissions and improve quality of life.

Main Duties of the Job
The Community Matron undertakes advanced clinical assessment, identifying needs, planning, case management, and coordinating care for patients with highly complex needs and co-existing long-term conditions. They work in partnership with other professionals and agencies to improve patient outcomes.
The post holder will provide clinical leadership for staff within the Community Nursing service, supporting the development of staff roles with regular training and supervision and will contribute to the assessment of competence for registered and non-registered staff.
Job Responsibilities
* Clinically supervise and demonstrate specialist community healthcare knowledge and skills to develop the skill set of community nursing staff, students, patients, and carers within a cluster of community teams.
* Role model compassionate and inclusive leadership where team members are supported to work and develop clinical practice across all localities, acting as joint supervisors to all Nurse Practitioners where needed.
* Undertake advanced clinical assessment, needs identification, planning, case management, and coordination of care for individuals with highly complex needs and co-existing long-term conditions.
* Work towards advanced professional practice, including advanced clinical skills, medicines management, leadership, and supporting self-care management, while forging interagency and partnership working to improve outcomes for patients.
* Proactively reduce unplanned hospital admissions and facilitate earlier discharge from hospital for patients identified through multi-disciplinary team meetings or proactive care management.
Person Specification
Qualifications / Professional Registration
* Registered Nurse 1st level
* First level degree in a health-related subject
* Advanced Physical Assessment and history taking qualification
* Independent Prescribing qualification or the ability to achieve this within 2 years of appointment
* Registered mentor
* Relevant and additional post-registration e.g., Advanced / Specialist qualification
Experience and Knowledge
* Specialist knowledge and experience in Community Care to Master's level equivalent or evidence of working at a Master's level equivalent
* Significant post-registration experience, preferably with some experience in a community setting or a clear understanding of Primary and community care
* In-depth experience in the management of individuals with long-term conditions
* Evidence of being an innovative practitioner
* Knowledge of or experience in coaching and mentoring practices and tools
* Knowledge of or experience in Quality improvement tools, techniques, and methods
Other Requirements
* It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role. However, the Trust would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role.
