A Care Manager plays a vital role in coordinating and overseeing care services for individuals in need of support, typically those with mental health conditions and elderly people. Below is a comprehensive job description for this role:

Job Summary:
The Care Manager is responsible for coordinating and managing the delivery of high-quality care services to clients. This includes developing personalised care plans, working with healthcare providers, and ensuring the overall wellbeing of clients by facilitating the appropriate support. The role involves liaising with families, healthcare professionals, and other service providers to create a safe and supportive environment for the individual.

Key Responsibilities:
* Client Assessment and Care Planning:
* Conduct comprehensive assessments of clients’ physical, emotional, and social care needs.
* Develop and implement personalised care plans in collaboration with the client, family, and other healthcare professionals.
* Regularly review and update care plans in response to changing client needs.
* Care Coordination:
* Coordinate and manage day-to-day care services, ensuring high standards of care are maintained.
* Liaise with multidisciplinary teams, including doctors, nurses, social workers, and other care professionals, to ensure holistic care.
* Facilitate referrals to specialist services where necessary.
* Monitoring and Quality Assurance:
* Monitor the delivery of care services to ensure they meet the agreed standards and regulatory requirements.
* Conduct regular reviews and quality checks, ensuring care providers are following best practices.
* Address and resolve any concerns, complaints, or issues related to care provision.
* Support and Advocacy:
* Act as an advocate for clients, ensuring their needs, preferences, and rights are respected.
* Provide emotional support and guidance to clients and their families, offering advice on care options and services.
* Help clients and their families navigate social services and healthcare systems.
* Team Management and Development:
* Supervise, train, and support care staff, promoting a culture of continuous improvement and professional development.
* Participate in recruitment, training, and performance evaluations of care staff.
* Ensure staff comply with legal, ethical, and regulatory standards in care provision.
* Documentation and Reporting:
* Maintain accurate and detailed records of all assessments, care plans, and interventions.
* Prepare regular reports on client progress and service delivery for management and external agencies.
* Ensure compliance with all relevant legislation, including safeguarding and health & safety regulations.

* Strong understanding of care management practices, particularly in mental health and among elderly care or disability support.
* Excellent communication and interpersonal skills, with the ability to liaise effectively with clients, families, and healthcare professionals.

Qualifications and Experience:
* A degree or diploma in health or social care, nursing, or a related field.
* Previous experience in a care management or supervisory role within a healthcare or social care setting.
* Knowledge of relevant legislation and regulations, including safeguarding, mental capacity, and health & safety.
* Experience in working with mental health, elderly or disabled individuals is preferred.
* Relevant care management certifications (e.g., NVQ Level 5 in Health and Social Care) are desirable.

* Enhanced DBS check.
* Flexibility to work outside of normal hours if required.
Job Type: Full-time
Pay: £25,000.00-£33,000.00 per year
