A rare opportunity for a full-time Community Care Coordinator in the Older People's Community Mental Health Services. Based at Meridian House, you will join the AWCMHTe, covering the beautiful Aire and Wharfe Valleys.
Once inducted and confident, you will have the opportunity to work in an agile manner and from home on occasion.
Responsibilities:
* Act as care coordinator for a designated group of clients in accordance with national and local guidelines.
* Construct, implement, and evaluate packages of care to effectively meet the needs of clients and their carers.
* Undertake accurate, timely, and skilled clinical assessments, and ongoing reassessments, taking into consideration all social, physical, and environmental factors.
* Support clients and carers in their understanding of a diagnosis and its implications for the future.
* Maintain relationships with wards, departments, and all other agencies to initiate and facilitate excellent service provision.
* Help identify junior staff development and training needs to enhance service provision.
