Our client based in Torquay is a leading Housing Association and one of the biggest providers of high-quality housing in the charity sector. They also offer an Enhanced Community Recovery Service and require a Health Coach to join their busy team until the end of July 25, with a potential to extend further.
The role of Health Coach is to use an asset-based approach involving coaching individuals and small groups with the aim of enabling individuals to make sustainable healthier lifestyle choices. As a Health Coach, you will work in a variety of environments with a range of clients to support positive changes and life choices in order to promote sustained improvements in health and wellbeing. The role will involve carrying out assessments, creating individualised health plans, and, where required, undertaking physical health checks. Having a good understanding of local community assets and building positive relationships with a wide range of partners will be an integral element of the role.
Main Duties:
1. Develop and sustain effective relationships with customers referred to the service in order to promote co-development of action plans.
2. Provide support and intervention to customers that deliver positive lifestyle changes in relation to:
1. Healthy Eating
2. Exercise
3. Alcohol and Drug intake
4. Smoking cessation
5. Relationships
3. Aim to reduce national trends in ill health including:
1. Obesity
2. Physical inactivity
3. Dangerous levels of alcohol consumption
4. Smoking
4. Undertake physical health checks as required and take appropriate action as a result of the checks including referral to other services.
5. Use agreed assessment tools in order to identify the customer's needs and priorities.
6. Support customers to identify behaviours that affect their health and wellbeing and empower them to take ownership and responsibility.
7. Support customers to develop health plans and to make the changes they want to achieve.
8. Support customers to maintain positive changes and to identify and navigate any barriers/actions that may prevent sustainable change.
9. Deliver defined service-specific interventions that support a customer to achieve their defined action plan.
10. Identify any Risk/Safeguarding issues or concerns and act in accordance with local policy and procedures where appropriate.
11. Identify customers with more complex health and social needs and gain consent to refer to relevant services to meet their needs.
12. Support the uptake of wider population health checks and assessments that support early clinical diagnosis and monitoring rates of subsequent uptake.
13. Work with statutory and non-statutory services to set up reasonable adjustments for the delivery of health care enabling access for vulnerable groups (including screening, preventative health and specialist services).
An NVQ Level 1 in Health & Social Care would be desirable and a recognised qualification in Health Training would also be desirable.
The role has a flexible approach, and you must be willing to work some weekends on a rota basis. An enhanced DBS is also required for the role.
