An exciting opportunity has arisen for a passionate and dedicated Pharmacist to lead the pharmacy team within the Frailty Unit at Morriston Hospital. In this role, you will work closely with a multidisciplinary team to optimise medication therapy, enhance treatment outcomes, and improve the quality of life for frail and elderly patients. As a key member of the team, you will also take an active role in prescribing for elderly patients during ward rounds. You will be responsible for managing the pharmacy service within the frailty speciality and will deputise for the lead pharmacist in medicine when required. You will receive full support and mentorship to help you succeed and develop in this exciting new role., As the lead pharmacist for the Frailty Unit, you will:
* Actively participate in ward rounds, ensuring the safe and effective use of medicines throughout their hospital stay.
* Offer ward-based medicines management services from admission to discharge and optimising medication use for frail and elderly patients by actively prescribing and de-prescribing medications
* Collaborate with clinicians, nursing staff, pharmacy colleagues, and other multidisciplinary team members to deliver high-quality pharmaceutical care.
* Provide day-to-day leadership and management of the pharmacy service for medical patients.
* Proactively provide evidence-based advice to prescribers to improve patient care and ensure cost-effective medicine usage.
* Engage in key directorate activities such as quality improvement and service redesign, medicines expenditure analysis, and risk management specific to elderly care.
* Supervise, mentor, and train junior colleagues, supporting their professional development.
* Participate in dispensary duties and offer clinical leadership to junior pharmacists and trainees. Contribute to weekend, Bank Holiday, and out-of-hours rotas as needed to meet the demands of the service.
The ability to speak Welsh is desirable for this post; both English and Welsh speakers are encouraged to apply.
