Are you passionate about mental health? If so, an exciting opportunity has arisen for a full-time Social Worker to join the Mental Health Social Work Hub. This bespoke adult social care team works closely with South West Yorkshire Partnership Trust Mental Health Services in offering a comprehensive, dedicated social care resource for people with mental health needs. The team will provide assessments, support planning, and commission services for people with mental health needs across the Wakefield District.
As a registered Social Worker, you will play a crucial role in empowering individuals with mental health needs to live independently and make informed decisions about their care. You will conduct assessments, support planning, and commission services tailored to each person’s unique needs. Experience in mental health is desirable, along with strong assessment and communication skills, and a solid understanding of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
As part of your role, you will undertake planned interventions, complete conversation-style assessments, strength-based reviews, and develop person-centred, and outcome-focused care and support plans.
What will be expected from me?
* Assessing and Meeting Care Needs: Ensuring that the care and support needs of adult citizens are met in line with national and local policy guidelines.
* Supporting Colleagues and Managers: Collaborating with health, social care, and voluntary and independent care sector professionals to deliver an integrated service.
* Providing Guidance and Advice: Offering expertise on matters related to vulnerable adults within the district.
Key Responsibilities:
* Manage a diverse caseload with the support of senior colleagues, applying a multi-disciplinary approach.
* Undertake planned interventions and statutory casework.
* Promote collaborative working with allied health and social care professionals.
* Prepare comprehensive reports on clients in accordance with policies and procedures.
Essential requirements:
* Degree in Social Work, or relevant Social Work qualifications recognised by the Central Council for Education & Training in Social Work/Registration with SWE.
* Evidence of or willingness to successfully complete post-qualifying award or training.
* Evidence of or willingness to maintain skill and knowledge base in professional field through post-qualifying training or other opportunities.
* Ability to develop and sustain effective working relationships with service users, carers, and groups.
* Excellent communication and engagement skills with a commitment to improving service delivery.
