If you have knowledge of services (both statutory and third sector) which are available within Denbighshire communities, then this role could be for you Our team of Community Navigators in Denbighshire provide a link between Health and Social Care professionals, Denbighshire citizens, their family, and carers. As part of the team, you would provide assistance within the community and third sector. This approach can also be known as ‘Social Prescribing’ which is a mechanism for linking citizens with non-medical sources of support within the community. You will recognise that people are experts in their own life but sometimes they need someone else to help them to identify and work towards the outcomes that they would like to achieve. A day in the life of a Community Navigator: You will work closely with the Community Resource Teams, ‘Talking Points’, SpoA Wellbeing Coordinators, Complex Disability Team, Primary care, Community Mental Health Teams and other people from a range of local services. You will engage with service users of Talking Points in their normal place of residence or perhaps at another setting. You will carry out needs assessments and develop support plans utilising the “What Matters” approach. You will be a source of current, accurate and timely information and knowledge about a comprehensive range of help available within the community. You will improve the service quality for the benefit of service users. To be a successful Community Navigator: Are you proactive, can react quickly to service requests and appreciate the importance of the service you are providing? Can you deal with queries in a diplomatic, confidential and non judgemental manner? Are you comfortable planning your own workload and working remotely whilst still part of a team? Do you have excellent communication skills, both written and verbally? Are you IT literate, particularly with Microsoft 365 packages?
