Senior Children's Community Nurse
NHS AfC: Band 6
12 Month Fixed Term Contract
We are seeking a dynamic and motivated Senior Nurse to join our Children's Community Nursing Team in the Community of St Helens and Halton areas. You will be based in Lowe House HCRC in St Helens and will be expected to travel across the boroughs.
There will be lots of opportunities for learning new skills; the successful candidate must be comfortable in working in patients' homes, liaising with other hospitals and preparing care plans for children in the community.
There will also be opportunities to work directly with complex needs children within the school setting.
An Enhanced DBS with barred lists for Children is required for this post.
Main duties of the job
* Post holder to work flexibly across the Children’s Community Nursing service, delivering high quality, evidence based nursing care for continuing care children (ICB commissioned packages of care) and young people with short and long term conditions including supporting children with life limiting and life threatening conditions.
* To liaise with the staff in the school regarding any changes in the care plans of the ICB funded children and young person.
* To keep up to date with members of the MDT team and with families, parents and carers.
* To accept responsibility for own professional practice as a reflective, accountable clinical practitioner.
* To provide nursing interventions to meet the complex medical health needs of young persons in school including supporting the young person on transport to and from school.
* To work within the Children’s Community Nursing Service, delivering high quality, evidence based nursing care for children and young people with short and long term conditions, including life-limiting and life threatening conditions that receive continuing care funding.
* To meet these health needs of the identified children by working in liaison with multi-agency partners to promote collaboration and effective communication.
* Please note that driving, and being a car owner is an essential requirement for this role as clinical interventions are undertaken in the community.

Detailed job description and main responsibilities
KEY DUTIES
* Post holder to work flexibly across the Children’s Community Nursing service, delivering high quality, evidence based nursing care for children and young people with short and long term conditions including supporting children with life limiting and life threatening conditions, preventing hospital admission and facilitating early discharge.
* To facilitate specific training and support strategies for those personnel involved with children and families, with a variety of healthcare needs, whilst also acting as a specialist resource for all colleagues, within educational and community settings.
* Participate in the developments of the Children’s Nursing Team in line with best practice outlined within research, national guidelines and local governance arrangements.
* To actively participate within specific medical consultant led outpatient clinics within the geographical area.
* To support the Team Leader in the effective daily management of the patient/team caseload.
* To accept responsibility for own professional practice as a reflective, accountable clinical practitioner.
* Ensure effective line management of staff.

Qualifications
* Nursing degree level educated/working towards
Knowledge & Experience
* Sufficient number of 2 years’ post-graduate children’s nursing experience
* Trained and experienced in Children’s long-term ventilation
* IV therapy/TPN
* Experience of caseload management
* Safeguarding Children to Level 3
* Teaching ventilation to non-medical staff
* V300 non-medical prescriber.
* Experience working in a community setting
* Experience of working with clinical commissioning groups and Continuing Care
Skills
* Demonstrable high level of inter-personal and communications skills
* A commitment to improving patient services through an ability to sustain a clear focus on continuous quality improvement
* High level of work organisation, self-motivation, drive for performance and improvement, and flexibility in approach and attitude.
