Following the exciting development of our Multispecialty Virtual Ward in conjunction with our partners within Bradford District and Craven, we have a number of opportunities for roles within the service.
The Virtual Ward is an innovative service aimed at supporting people to receive care, support, and interventions within their own home. This improves the quality of care for individuals, reduces the length of time people stay in hospital, decreases the need for admissions, and ensures beds are available for acute admissions.
Main duties of the job
This is an opportunity to gain experience and shape the development of the new service for the future. The role involves working as part of a multidisciplinary team both within secondary care and the community, providing care at home.
The successful applicant will have specific responsibilities for assessing, planning, implementing, and evaluating nursing care within a community setting. The post holder will help lead and coordinate the clinical, managerial, and educational requirements of the clinical area, ensuring the highest standards of nursing practice and care are delivered as reflected by the Virtual Ward. They will ensure the safety and wellbeing of service users and work in a manner that promotes dignity and human rights through the adoption of person-centred care principles. The post holder will also deputise for the Virtual Services Team Manager and act as a role model for junior team members in the delivery of high-standard nursing care. A full driving license is essential for this role.
