We are currently working in partnership with a Local Authority in the Blackburn Area who are seeking an experienced Leaving Care Personal Advisor to join their Leaving Care Service As a PA, you will play a crucial role in providing support and interventions to young people and young adults aged 16-25 years who are in, or leaving, care. Responsibilities: Provide practical, emotional, and aspirational support to young people and young adults, tailored to their individual needs. Work collaboratively with a diverse and cohesive team of Personal Advisors, fostering a positive and empowering environment. Conduct comprehensive assessments of needs for individuals post-18, developing co-created pathway plans based on the assessment outcomes.
