Anexciting opportunity has arisen to join the North Worcestershire CommunityLearning Disabilities Team as a Band 5 to 6 (developmental role) CommunityLearning Disability Nurse based at Princess of Wales Community Hospital,Stourbridge Road, Bromsgrove.
The successful applicant will becomepart of a small but committed team who are passionate in ensuring thatindividuals with a learning disability receive equitable health care, alongsidemanaging an effective caseload and maintaining regular visits to service usersand their carers.
The team is currently involvedin exciting service developments, including RcPsych QNLD Accreditation, STOMP,Dementia pathways, and transitions pathways to better meet the needs ofindividuals with learning disabilities.
Regular individual and groupsupervision and support will be provided as well as on-going CPD and trainingto help further develop your skills and competencies in this specialist field.

Main duties of the job
Actively manage an effective caseload,maintaining regular visits for service users.
Facilitate/teach effective systems tocarers to enable the transfer of skills required for the management of serviceusers healthcare needs.
Facilitate Health Action Plans for eachservice user on caseload, to enable access to the full range of primary andsecondary care services available.
Promote and maintain good workingrelationships and systems of effective communication through liaison withservice users, carers, and other professionals / agencies.
Maintain accurate and up to dateclinical records in line with Trust/NMC standards of practice.
Record activity to meet the requirementsof Patients Information Management System.
Attend team and multi-professionalmeetings as appropriate.
