We are seeking an Advanced Clinical Practitioner to join our medical team at Camborne & Redruth Community Hospital.
You will work alongside our GPs & ACPs assessing & managing patients presenting to our Same Day Emergency Care unit (SDEC) & those who have been admitted on Cornwall's Virtual Ward.
SDEC provides an alternative to the acute hospital for patients who can be safely managed outside of the emergency department, aiming for home discharge on the same day. The follow-up is provided by our Virtual Ward, where patients receive regular nursing contact & medical oversight 7 days a week. This is a Consultant-led service with opportunities to join ward rounds & MDTs for learning & service provision.
We also host a Urgent Treatment Centre & Community Assessment and Treatment Unit (CATU). The post holder may rotate through these services depending on skill & service requirements. The UTC sees a range of medical problems covering all age groups, self-presenting to the unit. The CATU provides short-stay, ward-based medical and nursing care, with therapy assessments.
The role will be varied and challenging but well-supported by our existing GPs. There is time and scope for leadership, personal development, and project opportunities. The role can have an element of bespoke creation while supporting us to deliver safe medical cover to our patients across these services.
This role will suit an experienced ACP, comfortable with working independently.
Key Responsibilities:
1. History-taking, examination, requesting investigations and creating an appropriate safe medical management plan for patients presenting to the services above.
2. Request and interpret investigations such as blood tests and x-rays.
3. Prescribe appropriate medications to support the patient's care.
4. Utilise all alternatives to admission services to enable people to be treated in their own homes whenever possible.
5. Work closely and collaboratively with your team of the day, with a 'can-do' 'yes' approach to your working.
We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.
We are a people organization and people matter to us. As part of the team, you’ll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.
Over 4,000 people make up the Trust, including doctors, nurses, therapists, and admin and support staff. We work in people’s homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work, and a great partner.
Approximately 568,000 people live here. A third of the population of Cornwall is supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer and during other holidays, many people choose to visit the area, increasing the number of people who use our services.
