Job summary The post holder is an experienced Pharmacist, who acts within their professional boundaries. They will work as part of a multi-disciplinary team in a patient-facing role. In this role they will be supported by a Senior Clinical Pharmacist and GP who will develop, manage, and mentor them. The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will respond to daily anima queries from patients, action medicines reconciliation on transfer of care and clinic letters. They will undertake medication reviews across different disease areas. Clinical Pharmacist- Prescriber: The post holder will be an independent prescriber and us such will be expected to work within their scope of practice and develop this scope of practice to meet the requirements in primary care. Clinical Pharmacist- Non -Prescriber: The post holder will be supported to develop their role to become a independent prescriber.
