Job summary Forum Family Practice are looking to recruit a friendly, motivated, enthusiastic and experienced care navigator/data administrator to join our team. You will play a crucial role in helping our patients both face to face and over the phone with their health care needs. The post holder will ideally have previous GP practice or NHS experience with knowledge of Systmone, however this is not essential. If you enjoy a variety in your work, have IT skills along with excellent customer service then this is a perfect job for you. We are looking for a minimum of 28 hours per week. Main duties of the job To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practices multidisciplinary team Process patient requests for appointments. Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately. Initiate contact with and respond to requests from patients, team members and external agencies.
