Registered Nurse/Social Worker/Registered Allied Health Professional? Interested in a new challenge using your excellent assessment skills? We are looking for qualified Senior Practitioners to join the Personalised Healthcare Commissioning (PHC) service to work with our Teams in supporting ICBs in providing support for patients in receipt of health funding.
The post holder will play a key role in supporting the Personalised Healthcare Commissioning service across the region. Working within the remit of the National Framework for Continuing Healthcare and other NHS funding streams, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.
You should be well versed in dealing with complex and sometimes contentious information.
The post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.

Main duties of the job
The post holder will be expected to lead on Continuing healthcare assessments across the responsible Integrated Care Board (ICB) area.
You will support and advise colleagues in relation to NHS Continuing Healthcare Funding ensuring current practice is being embedded into clinical practice, promoting best practice and compliance with the Department of Health processes.
The role will be responsive to ICB priorities and expectations whilst working on their behalf and representing them through work with NHS Continuing Healthcare.
Job responsibilities
* Working within a team of Personalised Healthcare Assessors, taking a lead role in complex cases, ensuring a high-quality service is delivered, supporting and mentoring staff to achieve continuous improvement.
* Managing a caseload of patients and contributing to performance improvement, taking a lead for identified areas, where agreed.
* Managing a defined caseload for those with complex health needs.
* Facilitating and contributing to multi-disciplinary assessments and patient reviews, in line with standard operating procedures.
* Supporting the PHC team to ensure workstream plans are fulfilled and supervised effectively to ensure successful delivery.
* Deputizing (as required) in the absence of a team leader for PHC.
* Assisting the Team Leader for PHC with resolution of complaints, appeals and disputes.
* Raising issues of concern through the Senior Management Team where necessary.
* Mentoring and supervision of PHC Associate Practitioners, Practitioners and students.
* Contributing to performance improvement within the team of Senior Practitioners, Practitioners and Associate Practitioners ensuring that services are delivered to a high standard.
* Monitoring workstreams of PHC Practitioners and quality assurance of processes.
* Working with members of the team to investigate the causes of any variance from target/plan and proactively contribute to the implementation of solutions.
* Actively supporting the PHC team in working towards goals and objectives.
* Communicating information, risks, issues and dependencies, including briefings and reports.
* Maintaining professional registration and up-to-date knowledge of legislation.
* Providing relevant and timely specialist advice and guidance on functional and information matters relating to the service.
* Delivering against objectives, achieving quality outcomes, prioritizing own workload and working to tight deadlines.
* Fostering transparency and a person-centered approach with patients, families and representatives.
* Promoting the use of Personal Health Budgets for eligible individuals as a means of their having control over their care and achieving best possible outcomes whilst being cost effective and value for money.
* Ensuring the principles and duties of safeguarding are holistically, consistently and contentiously applied.
* Contributing to maintenance and monitoring of service standards around quality.
* Complying with NHS Codes of Conduct and Standard Business Conduct and/or NHS Trust Standards of Business Conduct policies.
* Responsible for ensuring mandatory training is up-to-date.
Person Specification
* Extensive knowledge of specialist areas acquired through experience and training, plus further specialist knowledge of the continuing healthcare process and MDT working.
* Negotiation and conflict management skills and the ability to influence.
* Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales.
