We have 4 small friendly teams across Lincolnshire comprising of Occupational Therapists, Physiotherapists, Nurses, Pharmacy Technician, Promoting Independence Therapy Assistants and Promoting Independence Care Assistants. We are a growing team and looking for enthusiastic, friendly, and proactive therapists.
In our Pathway 1 Team, we follow the patient home and see them within 2 hours of their discharge. We listen to the patient of what matters most to them with regard to their personal and social circumstances, their physical, mental and emotional health and their carer's needs. This will include exercise programmes for their physical and mental wellbeing, recommending and providing the correct equipment, proactive work with patients to encourage independent living, educating patients on self-management of their health and wellbeing such understanding frailty symptoms and the importance of staying well physically and mentally for falls prevention, and so much more., Being part of our Team, you will be working closely with colleagues across all sectors when supporting patients in the community ensuring a high standard of therapy provision. You will have opportunities to be involved our Quality Service Improvement Programme and we welcome new creative, resourceful, and innovative ideas, giving you personal ownership in projects striving to do better.
