Job summary We are seeking a Discharge Coordinator to work within the Patient Flow team at Littlebrook Hospital working with Younger and Older Adults. The Patient Flow team is responsible for coordinating both early and complex discharges across the trust, minimising delays and maximising bed capacity. We are looking for an individual with a good understanding of acute inpatient carewho will be committed to driving the discharge process, identifying and anticipating any issues that may arise, working closely with the discharge coordinators and the MDT to develop effective relationships with both internal and external agencies. Encouragement will be provided to develop your skills, knowledge and competence through regular supervision, training and continuing professional development. What is High Cost Area Allowance? The High-Cost Area (HCA) allowance, also referred to as London Weighting is a payment made to employees who work in London and the surrounding areas. As this post is based in Dartford, Gravesham or Swanley, the successful applicant will receive an additional payment each month. 5% of basic salary, subject to a minimum payment of £1,258 per year and a maximum payment of £2,121 per year (the agreed annual amount will be divided into 12 equal amounts and paid monthly) Main duties of the job To work as a core member of the Patient Flow team in their locality area and contribute to discussions around discharge planning. To work with the MDT to identify, in a timely manner, those patients who have barriers to discharge. To work closely with patients and carers to understand barriers to discharge and to explore housing options with them. To participate in individual patient meetings and represent Patient Flow in these meetings. To support bed managers with bed requests by forming part of a call handling rota.
