Job summary We are looking for a highly motivated individual to join our team on AFU. The aim of AFU is to deliver high quality comprehensive geriatric assessment in the first 48 hours of emergency care to improve patient experience whilst reducing admissions and inpatient length of stay Your role will be key in facilitating the assessment of patients and co-ordinating a plan of care alongside the wider MDT. This post will involve working closely with ED to support the timely transfer of patients to the unit to ensure a CGA is commenced. Your leadership skills and expertise are essential to ensure the patient pathway for a patient living with frailty is holistic and patient centred whilst minimising the time spent in an acute setting. Our nursing team is supported by a team of Consultants Geriatricians, Advanced Clinical Practitioners, Therapists and Pharmacists who are committed to working with the wider multidisciplinary teach to continuously improve patient care.
