* Patient Care & Treatment:
o Provide comprehensive nursing care to housebound patients, ensuring dignity and respect at all times.
o Implement and review care plans tailored to individual patient needs.
o Administer oral, IV, and intra-muscular medications while monitoring patient reactions.
o Perform wound management, including compression bandaging and diabetic foot checks.
o Manage PICC and Hickman lines, including ongoing care and maintenance.
o Conduct phlebotomy, cannulation, and immunisation, including seasonal flu vaccinations.
o Perform catheterisation (male, female &Suprapubic)
o Removal of stitches and clips.
* Preventative & Hospital Avoidance Care:
o Participate in hospital prevention schemes, providing home-based treatment for conditions such as COPD to reduce hospital admissions.
o Collaborate with multidisciplinary teams, including GPs and social care professionals, to ensure coordinated patient care.
o Advocate for patients by ensuring their health and social care needs are met within the community setting.
* Safety & Compliance:
o Adhere to strict medication safety protocols, ensuring correct dosages before administration.
o Maintain accurate patient records and documentation in line with NHS guidelines.
o Follow infection control procedures and best practices in community healthcare settings.
Skills & Requirements:
?Registered Nurse (RN) qualification NMC registered
? Proven experience in community nursing, home care, or similar setting
? Strong clinical skills in wound care, catheterisation, IV therapy, and phlebotomy
? Excellent communication skills for liaising with patients, families, and healthcare teams
? Ability to work independently and manage a small caseload of patients
? Licensed driver (as home visits may be required)
