Care Navigation within a GP surgery is varied, interesting, and sometimes busy frontline work. If you have great communication, organisation, and IT skills, can remain calm under pressure, and prioritise your work efficiently, then this could be the role for you.
Previous experience is desirable, but full training will be provided for the right candidate.
Working pattern will be 10.5 hours per week, to be discussed at interview. The successful applicant will be expected to cover a reasonable amount of staff leave and sickness, sometimes at short notice.
PAY - £11.50 per hour.

Main duties of the job
Care Navigation requires daily face-to-face, telephone, and online communication with patients, to signpost, arrange appointments, and follow-up care.
You will be trained and responsible for directing patients to the most appropriate service or healthcare professional in a compassionate and timely manner.
You will also be expected to assist the team with administration tasks and build relationships with partner services.

Job responsibilities
Administration
* Process incoming mail
* Maintain and update patient records
* Photocopying, printing, and scanning to patient records
* Follow tasks requested by GPs and other healthcare staff
* Oversee practice emails
Reception
* Communicate with patients via face-to-face, telephone, and electronically
* Triage patient needs and navigate to appropriate healthcare services
* Manage online patient messages and requests via Practice website
* Process appointment requests
* Hand over any outstanding information and duties to colleagues as appropriate
* Flexibility to cover reception absences as required
Practice
* Ensure building safety at end of day
* Maintain tidy reception and waiting areas
* Maintain and update patient information boards, including waiting area TV
* Develop efficient and friendly professional relationships
* A thorough knowledge of practice policies and procedures
* To work in accordance with practice pathways and protocols
Person Specification
Qualifications
* GCSE or equivalent grade A-C English and Maths

Experience
* Good oral and written communication skills
