We are looking for dedicated Social Workers to join our Adult Community Team.
As a Social Worker in our Adult Community Team, you will conduct thorough Social Care Needs Assessments to understand and address the needs of adults in the community. You will develop and implement effective support plans, coordinate and manage services to meet individual needs, and assess and manage risks, including attending MARAMS. Your role will involve safeguarding vulnerable adults, applying Mental Capacity Act (MCA) principles to support decision-making, handling legal aspects related to the protection of adults through the Court of Protection (COP), ensuring compliance with Deprivation of Liberty Safeguards (DOLS), and utilizing the Decision Support Tool (DST) for Continuing Healthcare (CHC) assessments. You must be willing to work contracted hours within agreed rota patterns, including evenings and weekends as required.
Our busy community-based social work team is passionate about working with adults, carers, and partner agencies to achieve the best outcomes for those with care and support needs and their carers.
Minimum Requirements: Applicants will need to hold a social work qualification and have a professional registration as a social worker with Social Work England.
